Hospital to Home Transition Care

• Our program provides essential post-hospitalization support, designed to relieve the significant pressure and confusion families experience following a patient’s discharge. We help bridge the gaps in navigating a new set of temporary or long-term challenges.
• Package Value:
• We ease the burden of a smooth transition by providing personalized support. A compassionate care manager will meet your family either at the hospital or at their home to facilitate the process.
• Assigned caregiver: a dedicated professional is assigned to help navigate initial challenges, organize medications, assist with meal prep, and provide emotional support.
• Short-term assistance: daily check-ins for two hours a day over the next 3 days to ensure well-being and assist with immediate needs.
• Optimal Engagement Timing:
• When your loved one is scheduled for hospital-based surgery, or has been diagnosed with a chronic or acute illness.
• Cost: $2,000 one-time fee (after-hours travel fees may apply).

Hospital Escort

Unfortunately, most hospitals only provide basic service these days. We can accompany you or escort your loved one and ensure proper care during the visit.

Toileting, Bathing & Personal Care

We have licensed care givers who can preform these specialized tasks.

Hospital to Home with Nurse

• Our specialized Post-Discharge Concierge Service is delivered by a member of our nursing team and can address medical issues including wound care, pain management, chronic disease-specific care, and medication management.
• Cost: $2,500 one-time fee (after-hours travel charges may apply).
